Transfer requests are subject to campus capacity constraints. Overton ISD has the sole discretion to determine whether space exists at requested transfer site. Transfer applications will be for the current school year only and must be resubmitted for the next school year. Complete the form and include a copy of proof of residence, most recent report card, attendance report and discipline report from your child's most recent campus. (Proof of residence is current utility bill, lease, or mortgage statement)

These items may be uploaded and submitted with this form. If you prefer, you may also download or pick up a hardcopy of this form and submit it with the required documentation to the Overton ISD PIEMS coordinator located in the Overton ISD high school building.

Instructions for Completing Transfer Request Form

Use the Texas School Directory for TEA county-district numbers found at askted.tea.state.tx.us. For questions or concerns, contact the Overton ISD PIEMS Coordinator at 903-834-6144 option 3, or the Adminisration Annex at 903-834-6145.

Step 1: Fill out online Transfer Request OR Pick up Transfer Request Form at the Administration Building or download from district website at www.overtonisd.net. Step 2: Complete ALL parts of Section 1, Section 2, Section 3, and Section 4. Step 3: Upload supporting documentation and submit the form OR Take the completed form and documentation to the PIEMS Coordinator at the OISD High School Building or to the Administration Annex.

Step 4: Overton ISD will notify you of the decision. Until you receive notification from Overton ISD, the student cannot enroll on the new campus.

Use a different form for each student, and enter the student's social security number.
